1. Quality Manual: Address in Quality manual (clause 5.5 ) say that “Quality Assurance manager is acting as a Management representative and his/her responsibility and authorities are;
Ensure that the Quality Management System is established, implemented, maintained and continually improved;
Promote awareness of regulatory and customer requirements throughout the organization;
Report to the top management on the efficiency and performance of the quality system.
Liaison with external parties as applicable, on matters relating to the Quality Management system
2. Updating on companies organizational chart.
3. Job description: Address roles and responsibilities of management representative along with Quality assurance Manager’s activities.
4. Formal appointment Letter: The same way of above stated by Steelmaiden, the formal letter say that “XXXXX person has been appointed as a management representative, he/she will be responsibility and authority to implement and maintain the quality management system and report to top management on the performance of the QMS. The appointment comes in to effect from DD/MM/YYYY and the letter signed by CEO
This will be help to show the objective evidence to the auditors during the audits.

I am a Management representative of large organization, and my appointment letter says the same thing. that I have been appointed the responsibility and authority to act as the management representative of XYZ Company" and was signed by the MD.
We have addressed this through inclusion in three specific documents:
1) The Management Responsibility work instruction, that, in part reads:
"The Chairman & CEO appoints as the Management Representative the Quality Assurance Manager who, irrespective of other responsibilities has established authority over and responsibility for ensuring that Quality Management System requirements are effectively established in accordance with appropriate regulations and standards, and reporting on the performance of the Quality Management System to MWER for review".
2) The top level organization chart with a depiction of the "Management Representative" function reporting to Senior Management.
3) An identified reponsibility in the Quality Assurance Manager job desription.
All this having been said. a real piece of paper with real names and live signatures certainly couldn't hurt things. the "belt and suspenders" approach, if you will. ; ^ )

Not part of the "Quality Organization", no. Someone has to perform the duties, irrespective of their other roles and responsibilities. It may be better if they didn't come under the Quality org, IMHO.

Define the authorities delegated from the CEO to the QM so the QM can fulfill his or her responsibilities and be held accountable.
These authorities and responsibilities vary according to the needs of the organization.

An email send out saying that xyz person has been allocated the role with their duties being as defined in ISO 9001 and OHAS 18001
As Jane B was getting at, there is no requirement for an appointment letter, but as in our case, its a > 'expectation' . when they get it pointed out to them that an appointment letter is not required by these standards so cya is prudent.
I recently changed organizations.
When I arrived at the new employer, I read the Quality Manual. There is named someone specific as the MR. So we revised the manual, as the person left. The manual says now the "Quality Manager" will be the MR.
Then, the president of the company wrote the best change in MR letter I have ever seen:
To whom it may concern.
Effective immediately, Kronos147 is appointed Quality Manager and is assigned as management representative for the company.
He replaces the last person who has resigned his position following the completion of the surveillance audit conducted by Registrar on June 24th and 25th..
Kronos147 will handle all QMS issues.
5.5.2 Management representative
The President has assigned the “Quality Manager” as the “Management Representative” and irrespective of other responsibilities, shall have responsibility and authority that includes:
a) ensure that processes needed for the quality management system are established, implemented and maintained,
b) maintains the performance of the quality management system and any need for improvement, and (see 5.6 Management Review)
c) ensure the promotion of awareness of customer requirements throughout the organization, and
d) the organizational freedom to resolve matters pertaining to quality.
e) to have all necessary arrangement to facilitate all audit activities (3rd party certification, customer, regulatory entities and all internal/supplier audit), share audit reports, when available; this is to ensure ESM full commitment to compliances & certification with AS9100
The Management Representative has been identified within the Organizational Chart, Responsibilities and Authority List.
The Management Representative is responsible for addressing any issues or concerns during our Management Review Meeting (see 5.6 Management Review).
What I really like about the letter is I attached it to an e-mail and sent it out to our customers as my introduction. AS9100 may not say you have to do this, but I bet your customer's T's & C's say you do!
